By making this request I declare the following:

  • I authorise the person named in this form to borrow Library items on my behalf.
  • I understand that I am responsible for the library material borrowed on my behalf by my support worker or carer.
  • I understand that my support worker or carer must carry my MMU ID card and their MMU Library issued support worker / carer authorisation card, in order to be admitted into the library and borrow library materials on my behalf.
